The contents of the form is summarised below:

1. Definitions:
   • Data: means all data downloaded from the ftp website of the Left atrial wall challenge website for the workshop "Statistical Atlases and Computational Modelling of the Heart" 2016 held in conjunction with MICCAI 2016.
   • Recipient: means a researcher who has previously completed a Data Access Agreement form we sent to them, and has reeceived acknowledgement of its acceptance.
2. Purpose
   • The purpose of disseminating the Data to perform a multi-institutional analysis of a database of anonymised clinical MRI and CT datasets for Left atrial wall segmentation. This analysis is taking place in the context of the workshop on "Statistical Atlases and Computational Modelling of the Heart (STACOM)" 2016 held in conjunction with MICCAI 2016. The data is downloaded for this sole purpose and cannot be used in another context.
3. Confidentiality
   • The recipient(s) and the permanent staff signing this document commit to not disseminate the Data to any third party.
4. Retention
   • The recipient(s) and the permanent staff signing this document commit to delete their copy of the Data latest 24 months after the 1 April 2016.
